School, College, Sports and Camp Physicals

School, Sports, College and Camp Physicals are required at least once a year or before students participate in any physical activities. These physicals ensure that students are ready to play safely with no health issues.

Please gather all the necessary information and bring all relevant documents:

• Any required forms
• If students wear glasses, please bring those to the exam.
• List of allergies, current medications, medical conditions, past surgeries, if any
• Minors must be accompanied by a parent or an adult having parental/guardian permission.
• Letter of clearance from a primary care provider or pediatrician for medical or mental health conditions that might interfere with the child’s ability to play

We should be able to complete School, Sports, College and Camp Physicals in less than an hour!

Our medical provider will ask about your child’s medical history and perform a physical examination.
During the physical exam, our One Family Clinic & Urgent Care team will look for medical issues that may impact your child’s health when participating in an activity. We will assess your child’s height, weight, blood pressure, pulse, reflexes, and more. This exam is not an invasive appointment, and we will make the exam as simple as possible. Our provider will collect the information needed to fill out your physical form.
